Application Form for High School Seniors
The Department awards several scholarships each spring to deserving students—both
current students and those who are just graduating from high school. The deadline
for applications is always the second Friday of Spring quarter. The application
forms include instructions for how to apply.

BA

This form is a worksheet for students and advisers, It is useful in tracking
the completion of all major and GEC requirements.

Major
Program Form
One or two quarters prior to graduation, you should file your Major Program
Form with the Arts Advising Office. With this form, the department stipulates
that you have completed all requirements for the major, and describes any
deviations or exceptions that the department has approved in your major program.
Minor
Program Form
In the quarter before you graduate, you should file your minor program form
with your college advising office. With this form, you receive confirmation
that the courses you have taken in theatre comprise a qualified minor program.

This form charts the various prerequisites that are applicable to theatre
courses.
Program
Petition Form
The petition form is used by students who need to change or deviate from some
requirement in the major program. Petitions need to be presented to the Director
of Undergraduate Studies. Petitions are reviewed by the Undergraduate Studies
Committee. Directions for submitting a petition are found on the form.
